Here at WellSpine, we offer a wide array of Minimally Invasive Surgery procedures, including but not limited to: Artificial Disk Replacement, Pedicle Screw Fixation, Cervical, Lumbar and Thoracic Discectomy, Decompression and Fusion. Dr. Batlle is fluent in Spanish as well!
   
   
 


Initial Visit Instructions

We know that a visit to the doctor's office can be stressful, and we want you to feel as comfortable as possible from the moment you arrive. Many patients feel most relaxed when they are prepared and know exactly what is going to happen.

Here are some things you can do prior to your arrival at our office to enhance the experience. In order for us to treat your spinal condition or injury, we need to have a comprehensive understanding of your current health status. When you come in for your initial consult, please bring all of the items listed below. If WellSpine does not have all of your relevant medical records and films at the time of your visit, your appointment may need to be rescheduled.

Information to Bring:

  1. Previous x-rays, CT scans, or MRI scans: films AND reports must be provided
  2. Medical records of previous testing or treatment (EMGs, physical therapy, etc.)
  3. Any brace or support that you are using
  4. Referral(s) and pre-authorization(s)
  5. Insurance card and applicable co-payment

Please download and complete the following New Patient Forms prior to your visit:

Upon arrival at WellSpine, please register at the front desk, where the completion of all appropriate forms will be verified, and where you will be asked for personal and insurance information. You will also be asked to review and sign a Healthcare Proxy form.

Once registration is complete, you will be escorted to the first available exam room where you will change into a dressing gown. A Nurse Practitioner (NP) or Fellow will review medical history and perform a physical examination. A physician will then meet with you for an examination and consultation.

After your examination and consultation, you will receive an explanation of your condition and may be provided with instructions on necessary follow-up diagnostics or appointments. Your physician will discuss the range of treatment options for your diagnosis.

It may take as long as two hours to complete your initial visit and may be longer depending on the complexity of the care required.
